Keep the reason drop down list blank which makes the user drop down the list and choose. Also give the option to set the note mandatory or not.

I can see how keeping the reason dropdown blank by default (so users have to actively choose) and having a toggle to make the note mandatory vs. optional would make the workflow clearer.
